団体プログラム参加合意書
ワイカト大学カレッジ(UWC)の団体プログラムにお申し込みありがとうございます。
申込書に必要事項を正確にご記入ください。
ホームステイ体験はニュージーランドの生活・文化を知る上でとても重要です。そのため、必要事項を 正直に記入していただかないと、適切なホストファミリーをご紹介することができませんので注意してください。
また、現在かかっている病気や過去の既往症があれば、必ず申告して下さい。
申告を怠ったことにより、 プログラムの途中で帰国しなければならないということにもなりかねません。
研修期間中は必ず、所持品の盗難・ケガや病気治療、両方の保険に加入して下さい。保険に加入してい なければ、ニュージーランド滞在中にかかった医療費を全額支払わなければなりません。

◆ワイカト大学カレッジ（以下 UWC）の学生として、以下のことは必ず守ってください◆
・ 他者およびその所有物を丁寧に扱うこと
・ 事故もしくは病気になった場合、ホストファミリーと UWC の双方に連絡すること
・ ホストファミリー、ホームステイまたはプログラムに関する問題は UWC に連絡すること
・ 健康上の理由以外は授業にきちんと出席し、欠席する場合は UWC のスタッフの許可を得ること
・ コースを終了するためには最低 85％の出席率を確保すること
・ ニュージーランド国内の法令を遵守すること
～特に禁止されている項目は以下のとおりです～
・ 医師の処方箋なしの医薬品の服用
・ 18 歳未満のアルコール類の購入または公共の場での摂取
・ 20 歳未満で運転免許証を持たない場合のレンタカー手配
・ 16 歳未満、もしくは両者の合意に基づかない性的交渉
◆問題に気づかない限り、我々は支援することができません
ニュージーランド滞在中は、危険を伴うアクティビティまたはスポーツへの参加は控えていただくこと を強く勧めます。
また、アクティビティの参加前に、健康診断書の提出を求められることがあります。
以下のような事象があった場合、UWC は、UWC マネージャーの裁量で学生および団体の研修を中断 する権限を有します。
・ 公的不法行為 ・ 継続的な授業妨害
・ 大学の規則、ガイドラインの違反
・ その他、UWC が極めて問題があると判断した行為
私は、私の情報が UWC のスタッフおよびホストファミリーとで共有されることを理解します。
私は、私の健康・安全上必要であると UWC が判断した場合、UWC が私の両親に連絡することを許可 します。
私は、上記内容を読み、内容に合意します。

University of Waikato College
GROUP PROGRAMME
Thank you for applying to the University of Waikato College (UWC) Group Programme.
Attached you will find our enrolment form for the Group Programme. We would like you to complete this form accurately.
Your Homestay experience is important to help you to learn more about the life and culture in New Zealand, therefore if you do not complete this form honestly, we will not be able to match you to a suitable host family.
If you currently have or have had in the past any medical conditions, you MUST declare this information. Failure to do so may result in you being sent home from the programme.
We advise that you must have insurance to cover both your personal belongings and any medical treatment during your study tour. If you do not have any insurance to cover both of these items, you will be liable for the full cost of any medical treatment you may receive while in New Zealand.
Information on your immigration requirements and advice are available either through your agent or they can be viewed through the New Zealand Immigration Service website http://www.immigration.govt.nz .

Group Programme Student Participation Agreement
As a student with University of Waikato College (UWC) you must:
• Treat all others and their property with respect
• Inform your Homestay family and UWC of any accident or illness
• Inform the UWC of any issues you are experiencing with your host family, accommodation or your programme
• Attend UWC unless you are ill or have permission from UWC staff to be absent
• Attend at least 85 % of their class if to successfully complete the course
• Obey the laws of New Zealand
In particular, you must not:
• Take any drugs unless a doctor has prescribed them
• Purchase or consume alcohol in any public place unless you are 18 years of age or over
• Hire a rental car unless you are 20 years of age or over and have a valid driver’s licence
• Become sexually involved with anyone unless you are 16 years of age or over and both of you consent
We can only assist if we are aware of the issues.
UWC strongly recommends that the students do not participate in dangerous activities/sports while studying in New Zealand.
Students must be physically able and well enough to participate in any scheduled activities. The College reserves the right to ask for a medical certificate before students can join activities.
UWC reserves the right to terminate any students and group from their enrolment at the discretion of UWC Director. Termination may result from:
• Public Nuisance
• Secondary disruption in class
• Contravening University procedures and guidelines
• Any other action that the UWC deems significantly serious.
I understand that the information given will be shared with UWC staff and my Host family.
I give permission for the UWC to contact my parents when UWC judges it to be necessary for my welfare and safety.
If you do not understand any part of this form, please ask your school or your agent.
